Job Description:

Based in Oak Crossing Long Term Care. Job Duties include: new admission assessments, care planning, MDS completion, high risk charting. Collaboration with interdisciplinary team for best care of population. The incumbent must perform all duties in a manner that contributes to meeting the mission and goals of Essentia Health.

Long Term Care experience preferred.

No educational requirements.

Licensure/Certification Qualifications:

Registered or registry-eligible with the Commission on Dietetic Registration of the American Dietetic Association.

Must be eligible to be licensed with the State of Minnesota Board of Dietetics and Nutrition Practice
