LVN/LPN Patient Coordinator
General Summary: Primary responsibility of completing follow up calls on patients who are either on home health services or recently discharged from home health services and appropriately assessing their health status and any symptoms by voice alone.
Patient Population: Has contact with patients of all ages in the home setting; understands and demonstrates appropriate behavior when interacting with patients in all populations.
Essential Functions:
- Make patient calls on a wide range of topics as assigned.
- Ask the right questions to get the information needed to make an accurate assessment following an outlined script.
- Ability to carefully listen to the caller’s symptoms before developing a plan for further action/care services.
- Ability to hear not only what the patient says, but also have the ability to interpret such subtle indications as the tone of voice, hesitations or incomplete responses.
- Use a computer to access patient records and search the record for previous conditions and treatments.
- Evaluate patient’s status over the phone and determine if additional/changed care is needed, or if the physician should be contacted.
- Communicate with local agency management and clinicians or patient physicians as indicated.
- Provide clear patient information and symptoms to the clinical staff and/or physician as indicated.
- Work solely through the telephone majority of the time, but some may take on this task along with other in-person patient care duties as assigned.

Requirements
Supervises: N/A
Experience:
- One year of home health nursing experience.
- Current CPR certificate.
- Current driver's license in good standing.
On-going employment requires maintaining a valid RN/LVN/LPN license in the state where the nurse practices in and a valid CPR certificate at all times. The employee is responsible for renewing these both before they expire to continue employment.
Skills:
Extremely skilled listener with an ability to notice any subtle vocal indications or hesitations that could signal an issue.
Possess solid clinical judgment skills to develop the best plan for further treatment/care service needs.
Decision-making abilities are important to determine whether patients have a medical emergency correctly.
Have excellent computer skills to type information being given by patients.
Have good communication skills with compassionate interpersonal skills.
Be able to determine language/communication barriers and needs and access interpreter services as applicable to enhance communication with the patient.
Knowledge of Word, Excel, PowerPoint, and Outlook. Kinnser knowledge preferred.
Education:
- Graduate from an accredited college/school of nursing is required.
Licensure/Certification:
- Current and unencumbered State professional RN or LVN/LPN license.
Physical Requirements:
- Prolonged sitting is required.
- Ability to handle stressful situations in a calm and courteous manner at all times.
- Requires working under some stressful conditions to meet deadlines and Agency needs.
- Prolonged standing/walking is required.
- The ability to lift/transfer up to 50 lbs.
- Ability to meet deadlines and patient needs, make quick decisions, and allocate resources under stressful conditions.
- Meet patient/family individualized psychosocial needs.
- Requires hand-eye coordination and manual dexterity.
Environmental/Working Conditions:
- Works in a routine office environment.
- Ability to work a flexible schedule and extended hours as needed.
- Some exposure to unpleasant weather.
