Will work as a Psychiatric Consult Nurse

The goal of the Psychiatric Consultation-Liaison Nurse is to improve the care of psychiatric patients seeking care at the medical center for non-psychiatric reasons or those awaiting placement in a psychiatric facility.

  • Provide psychiatric nurse consultation services on the care of patients with a primary or comorbid psychiatric diagnosis. 
  • Assists the psychiatrists and psychologists in the care coordination of ordered psychiatric consultations. Gathers patient information and may document in the patient chart for the initial psychiatric evaluation and follow-up visits according to the current government regulations (e.g. Medicare, Medicaid, etc.). Is able to effectively communicate with patients and families during the consultation process. 
  • Coordinates level of care disposition or other recommendations made by the psychiatrist after a consultation. Arranges discharges to a recommended level of care, including transfers to inpatient, outpatient, or other community resources.
  • Following determination regarding level of care, completes insurance pre-authorizations.
  • Completes required documentation as necessary for transfer.
  • Maintains ongoing, frequent communication regarding each patient with hospital physicians and nurses, or other hospital personnel as needed. Has familiarity with community and system inpatient and outpatient resources for Behavioral Health patients. Works closely with hospitals and organizations community-wide in facilitating hospital transfers to psychiatric facilities and programs, as needed.
  • Participates in hospital meetings, hospital huddles, and other activities as requested by hospital administration or physicians.
  • Available to all hospital departments as a resource regarding Behavioral Health patient issues, assisting departments as necessary in difficult patient or family issues. Serves as a liaison between hospital departments and the psychiatric consultation liaison team.
  • Monitors and assesses medication regimen and assists with compliance concerns; escalates concerns to physicians.
  • Leads Behavioral Response Team calls.
  • Rounds on patients at risk for violence.
  • Assists with and/ or leads WPV event debriefing.
  • Provides education to clinical team on Behavioral Health diagnoses and treatments.
  • Rounds on all patients on violent restraints and works with team on interventions to discontinue restraints.
  • Coordinate and assist with Petition and Certificate or Voluntary Admission process, paperwork and filing to the court.
  • In coordination with nursing leadership, assists with and/or leads the implementation of evidence-informed projects, quality improvement projects, and the generation of new scientific knowledge with a focus on patients with a primary or comorbid psychiatric diagnosis or on teammate health/well-being.
  • Demonstrates a clinical working knowledge of Behavioral Health diagnoses and interventions.
  • Demonstrates knowledge of state and local community resources for Behavioral Health patients.
  • Demonstrates knowledge of IL Mental Health Code
  • Demonstrated ability to work closely with patients, physicians and other hospital personnel, requiring exceptional communication skills

Includes the patient and family in planning and implementing care and develops and maintains a therapeutic relationship throughout the healthcare continuum.
  • Anticipates and recognizes significant changes in the patient’s condition, status, and need for care. Provides ongoing assessment of the patient and takes appropriate actions when changes occur. Anticipates and alters the plan of care as needed, mobilizes appropriate resources, and collaborates with other health care team members to coordinate patient centered care.
