Department Overview
The transplant APP working as a member of an inter-disciplinary team is integral to Clinical Transplant Services. The APP assists in facilitating discharge for transplant patients, providing education, and works closely with staff members of the transplant team (RN's, physicians, social worker, financial counselor, psychiatrists, etc.) and has the primary responsibility for providing direct and indirect professional care for transplant patients both in the inpatient and outpatient setting.
