About the Role:

The Care Manager is an integral member of Community Reach Center’s AWARE Center Program. The Care Manager provides clinical and support services to consumers through integrated behavioral healthcare services and will utilize the Care Management Model that emphasizes the uses of basic community resources, referring, linking, and advocating, which will assist consumers in living as independently as possible in the community. The Care Manager provides care to consumers in a manner that recognizes the human potential, is empathic, and demonstrates unconditional positive regard while promoting safety, comfort, and therapeutic environment. Additionally, the Care Manager will have other duties and responsibilities as determined by the Program Manager.

Duties and essential functions: 

Provide care management services:

  • Provide an assessment of needs, service plan development, and work with consumers in the use of basic community resources, referring, linking, and advocating as needed. Acts as a liaison when necessary.
  • Provides prevention/early intervention, case management, family psychoeducation, and other clinical interventions to consumers, as well as therapeutic crisis intervention.
  • Facilitates and contributes to the accomplishment of tasks by consumers, empowers consumers to reach optimum level of achievement, and sets a positive example and provides role modeling as well as expertise to accomplish goals.
  • Gathers and maintains knowledge of community resources to better assist consumers.
  • Will work closely with outside organizations on resources and care coordination.
  • Support and closely coordinate mental health care with the consumer’s clinical care team and the primary care provider, supports, and family members as needed.
  • Provide brief interventions using evidence-based techniques such as Motivational Interviewing, or other treatments appropriate to support consumer in engagement in recommended services, in maintenance of housing, in making and keeping medical appointments and support healthy living activities.
  • Provide transportation for consumers in agency vehicles when clinically indicated.

Documentation:

  • Track patient follow-up and clinical outcomes using Electronic Health Record system (EHR). Document in-person and telephone encounters in EHR and use the system to identify and re-engage patients.
  • Ensures that member contact is documented, allowing adequate time for same-day documentation of member interaction.
  • Complete paperwork as required by Colorado or Community Reach Center.
  • Write and submit reports in a timely manner.

  • Uses identified agency key performance indicators (KPIs) to track success, engagement, and other factors leading to successful completion of treatment.
