About the role

This position will be working Monday to Thursday Day shift and primarily supporting our Milwaukie site but may require some traveling to other sites as needed.

This position is responsible for PT evaluations and development and implementation of the PT plan of care in accordance with PACE policies.  The PT is a member of the Interdisciplinary Team and responsible to communicate and coordinate care with the team as well as multiple outside providers, caregivers and family members.  The PT functions independently, working in multiple settings including the PACE center, participant homes, Adult Family Homes, Assistive Living Facilities and nursing homes.  The PT assumes responsibility for participants treated by the PTA and PT aide.

Required Qualifications:

  • Bachelor's Degree Physical Therapy
  • Upon hire: Oregon Physical Therapist License
  • Within 30 days of hire: National Provider BLS - American Heart Association

Preferred Qualifications:

  • Master's Degree Physical Therapy Or
  • D.P.T. Physical Therapy   
  • 1 year Working with a geriatric population

The amounts listed are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.
