Job Description for Certified Nursing Assistant

Position Summary:
The Certified Nursing Assistant (CNA) is responsible for delivery of personal care and minimal homemaker services (i.e. services promoting a safe and clean patient environment) to hospice patients in the home, assisted living facility, or long-term care facility. Services are provided based on Hosparus Health policies and according to the CNA plan of care (POC). The CNA also participates in the development of policies and procedures that guide the provision of hospice care.
Position Duties & Responsibilities:
• Demonstrates knowledge of all CNA forms, documentation methods, and procedures included in the care plan.
• Complies with regulatory agency standards and participates in regulatory survey process as requested.
• Demonstrates ability to identify information needed by team members to facilitate patient and family care.
• Participates in rotating call schedule to assure consistent coverage to meet r patients’ needs. Provides additional CNA services to patients as requested by RN.
• Collaborates with Clinical Manager and/or Clinical Leader as well as CNA schedulers to assure completion of duties, scheduling, and arranging coverage for CNA services as requested.
• Communicates via HIPAA compliant texting, or by telephone, with primary RNs and Clinical Manager and/or Clinical Leader as needed to assure patient care needs are met and documents interactions in the electronic documentation system (EDS).
• Collaborates with team members to assure coordination in delivery of care, which includes participation in Interdisciplinary Group (IDG) meetings and development of Interdisciplinary Team Plan of Care.
• Completes assigned plan of care then collaborates with patient/family and/or facility staff to ensure satisfaction with services and to review for potential POC changes that will promote quality care.
• Manages time efficiently and effectively; offers assistance with other organizational CNA needs as requested
• Notifies scheduling when completing assigned tasks early or when tasks scheduled during the scheduled work week may result in overtime.
• Maintains and submits daily activity logs by the end of each day via HIPAA secure texting.
• Evaluates utilization of all patient care resources (i.e. durable medical equipment, supplies) and communicates with primary RN, customer care or appropriate departments to ensure cost effective care.
• When paper documentation is used during periods of EDS downtime, the CNA submits forms to the community assistant on a daily basis.
• Demonstrates and delivers personal care per the CNA plan of care to hospice patients in the home, assisted living facility, or long term care facility as requested by and under the supervision of a registered nurse.
• Takes responsibility for ensuring the CNA plan of care is completed and necessary changes are communicated to the primary RN when appropriate.
• Reinforces patient/family education specific to the patient’s health care needs in ways that are understandable and appropriate.
• Reviews CNA plan of care prior to each assigned visit before providing care.
