Clinical Nurse Stroke Navigator (Hybrid-Remote)

IU Health Physicians Neurology has an outstanding opportunity available for an Ambulatory Nurse Navigator to support the Stroke program. This position works Monday-Friday, day shift no weekends, holidays or call! This role also allows for a hybrid-remote and in office work schedule.

The Vascular Neurology Nurse Navigator is responsible for guiding and supporting patients through all stages of their stroke and vascular-related care at IU Health. The Navigator proactively identifies and resolves patient needs and ensures immediate access to support and clinical guidance. The Navigator focuses specifically on improving transitions in care and adherence to care, particularly for high-risk patients or patients with complex care needs, to achieve optimal patient outcomes. A strong knowledge of stroke-specific care standards is required. Beyond direct patient care, key responsibilities also include program development, evaluation, maintenance, and improvement; research support and coordination; and participation in education initiatives. 

General Responsibilities:

• Provide a consistent point of contact for patients and their families
• Serve as a liaison between the program, referring and affiliate providers, clinic staff and other members of the care team
• Facilitate the scheduling of patient appointments, diagnostic testing, and procedures
• Schedule and manage hospital follow-up appointments for the inpatient vascular neurology team
• Participate in 7-day follow-up visits – assess and help address post-discharge needs and concerns 
• Develop and implement efficient systems to enhance patient satisfaction and provider productivity. 
• Interact with other departments, physicians, and staff to assure delivery of quality care to patients. 
• Support and/or participate in research programs

Telestroke Program Responsibilities

• Manage Telestroke Dashboard. Chart abstraction and data collection
• Education. Remote and onsite education about program components and operations
• Client support - act as a liaison, provide product/services information, answer questions, and resolve emerging problems
• Client Feedback – Collect and utilize feedback from clients to optimize the telestroke program. Curate and provide usage data highlighting outcomes and effectiveness to clients. 

Vivistim Program Responsibilities

• Schedule, track, and manage patients as they move through the Vivistim work-flow
• Manage Vivistim Dashboard. Chart Abstraction/Data Collection

 

The Clinical Nurse Navigator coordinates the care needs of assigned patients and develops comprehensive plans to manage care delivery across the patient care continuum. Partners with patients and their primary physicians to develop customized care plans based on their individual needs and preferences. Collaborates with physicians, nurses, health professionals, social work, and others to ensure appropriate tests and treatments are delivered in a timely fashion. Advocates for the patient. Balances care needs and financial considerations to ensure efficient and effective treatments are achieved.

• Requires an Associates of Nursing (ASN).
• Bachelors of Nursing (BSN) preferred.
• Requires an active Registered Nurse (RN) license in the state of Indiana or an active Nurse Licensure Compact (NLC) RN license.
• Requires that the RN has graduated from a nationally accredited nursing program. 3-5 years of experience preferred.
• Requires Basic Life Support certification through the AHA.
• Other advanced life support certifications may be required per unit/department specialty according to patient care policies.
• Specialty certifications may be required per unit/department specialty in accordance with the department provision of care.
• Requires knowledge of personal computers using various types of word processing and database software.
• Requires clinical work experience, medical terminology.
• Requires understanding of medical record requirements, regulations and policies.
