As the Associate Director of Care Coordination at Banner Desert, you have the opportunity to lead a team of 12-19 direct reports including social workers, nurses and non-licensed professionals. You will work with your team to help remove barriers to discharge for patients, provide education and feedback to direct reports, collaborate with other leaders, lead team meetings and education series.

Masters Degree in Social Work or Nursing background will be considered. Must be a licensed RN or LMSW/LCSW.

This is a full-time opportunity. Expected hours are Monday through Friday, between the hours of 7AM and 5PM. You will have a weekend on call rotation.

POSITION SUMMARY
This position provides managerial and clinical support to patients and staff. Uses depth and breadth of clinical management knowledge to ensure the provision of high quality, safe, and cost effective patient care. Demonstrates care coordination expertise and oversees the leadership of clinical, financial, and personnel management of the Care Coordination department. This position supervises employees and participates in selection, orientation, counseling, evaluation and staff scheduling. Maintains clinical, leadership and post-acute care services knowledge and competency to evaluate outcomes related to delivery of Care Coordination services.

CORE FUNCTIONS
1. Oversees Care Coordination personnel as assigned. Oversees professional development for social services staff as evidenced by continuing educational opportunities, case study reviews, and mentoring. Provides and/or facilitates Care Coordination and/or other related activities and serves as a resource to other staff members. Promotes utilization management through interdisciplinary patient care and activation of the discharge plan per the care delivery model.

2. Directs personnel actions including recruiting, new hire actions, interviewing and selection of new staff, salary determinations, training, and personnel evaluations. This position also participates in the development of Care Coordination goals and objectives in accordance with company standards. Ensures all goals and objectives are met timely and effectively.

3. Serves as a resource and provides leadership assistance to achieve optimal clinical, operational, financial, and satisfaction outcomes.

4. Supports and facilitates innovation and change management. Participates in the development, implementation and evaluation of the goals/objectives and process improvement activities within the department and organization. Analyzes data and healthcare trends to gain efficiencies and improve patient outcomes.
