Job Summary: The Baptist Health Deaconess Madisonville Remote Population Health & Transitions of Care Clinical Pharmacist practices exclusively in a virtual environment and is responsible for optimizing medication-related outcomes for high-risk patients using telehealth and population health tools. This pharmacist delivers comprehensive medication management (CMM), provides post-discharge medication reconciliation, addresses adherence barriers, and supports value-based care and quality metric performance—all through remote care coordination.
The role collaborates virtually with providers, care management teams, and outpatient partners to improve continuity of care, reduce readmissions, and enhance chronic disease management across the health system.

Responsible for optimizing medication-related outcomes for high-risk patients using telehealth and population health tools

Conduct post-discharge medication reconciliation for patients meeting organizational transitions-of-care (TOC) criteria.

Identify and resolve medication discrepancies, adherence barriers, polypharmacy concerns, and adverse drug event risks.

Performs comprehensive medication management processes

Support quality performance for programs such as HEDIS, CMS Star Ratings, ACO measures, and Medicaid quality programs.

Deliver comprehensive medication management (CMM) services for chronic disease management, including diabetes, hypertension, heart failure, COPD, and other value-based care priority conditions.

Engage patients through telehealth, phone calls, virtual visits, or in-person consultations depending on program design.

Performs other tasks/projects as requested.  Willingly accepts additional tasks/projects as assigned by an immediate supervisor or director.  Takes initiative to complete unassigned tasks without being prompted

Attends all required safety training programs and can describe his/her responsibilities related to general safety, department/service safety, specific job-related hazards, and national patient safety goals

Maintains compliance with state and federal laws and regulations related to the practice of pharmacy

Minimum Education, Experience, Training, and Licensures Required:

Required

• Doctor of Pharmacy (PharmD) degree from an ACPE-accredited institution.

• Active and unrestricted pharmacist Kentucky licensure.

• Completion of PGY1 Pharmacy Residency or equivalent clinical experience (3–5 years in ambulatory care, TOC, or chronic disease management) and sit for Board Certification within 18 months of position hire date.
• Excellent communication, education, and patient engagement skills.
• Proficiency with EHR systems and population health platforms.
• Ability to work independently, manage a remote caseload, and determine clinical priorities.

Preferred

• PGY2 Residency in Ambulatory Care, Internal Medicine, or Transitions of Care.
• Board Certification (BCACP, BCPS, BCGP, etc.).
• Experience working in remote care delivery, population health, or value-based care programs.
• Familiarity with collaborative practice agreements and pharmacist-run virtual clinics.
