Join Our Growing Rehabilitation Team

Lock Haven Rehabilitation and Senior Living is seeking a compassionate and motivated Full-Time Physical Therapist (PT) to join our dynamic rehabilitation team. This position offers the opportunity to work collaboratively with therapists, nursing staff, physicians, residents, and families to deliver high-quality rehabilitative care that promotes mobility, independence, functional improvement, and enhanced quality of life.

Responsibilities

  • Evaluate residents and develop individualized treatment plans based on clinical findings, functional abilities, and resident goals.

  • Provide skilled physical therapy interventions designed to improve mobility, strength, balance, coordination, and overall functional independence.

  • Monitor resident progress and adjust treatment plans as needed to support positive outcomes.

  • Accurately document evaluations, treatments, progress notes, and other required clinical information in a timely manner.

  • Collaborate with interdisciplinary team members to support resident care goals and discharge planning.

  • Educate residents, families, and caregivers regarding treatment plans, safety techniques, and strategies to maximize functional outcomes.

  • Maintain compliance with facility policies, professional standards, state regulations, and federal guidelines.

  • Participate in care conferences and team meetings as appropriate.

  • Ensure a safe and therapeutic treatment environment for all residents.

Qualifications

  • Graduate of an accredited Physical Therapy program required.

  • Current Physical Therapist license in the State of Pennsylvania in good standing.

  • CPR certification preferred.

  • Previous experience in skilled nursing, long-term care, rehabilitation, or post-acute settings preferred.

  • Experience with Net Health documentation software preferred.

  • Experience with PointClickCare (PCC) preferred.

  • Strong clinical, communication, organization, and interpersonal skills.

  • Ability to work effectively as part of an interdisciplinary team.
