The Medicare Population Health Programs Team is seeking a full time Medical Director to join our High Risk Care team in a National role supporting high risk programs for the (West/ Central - WA, AZ, CO) regions. The Regional High Risk Care Medical Director - (West/ Central) will serve as a key member of the High Risk Care team, playing a critical role in guiding the clinical management of our high risk/high need patients across markets. This role reports to the National Medical Director and VP, Medicare Population Health Programs.
Position Highlights:
- Accountable for market outcomes of admissions per thousand and readmission rate, as well as market-based program outcomes related to Part B, RAF, and Quality
- Attend and collaborate on high risk care team meetings (huddles, care coordination rounds) to guide clinical plan of care
- Collaborate with high risk team NPs and NP Medical directors on complex needs and clinical escalations, including calls to Specialists and hospitalists
- Collaborate monthly with market NPs on high cost patients, utilization review, intake reviews. Perform periodic chart review for clinical quality improvement
- Collaborate on the transitions management program for hospitalized and recently discharged patients, working with a RNs and the social resource team, as well as the Tele-Assist Team
- Review admissions and utilization data to identify patients for program inclusion in transitions and high risk care programs
- Clinical case review for transitions and specialist opportunities. Partner with the Network team around referral patterns, integrated program opportunities, and high-utilization patients
- Partner with market and district clinical leaders on program outcomes and performance, leveraging their interface with markets to reinforce implementation and uptake of other MPHP strategies and programs
- Partner with MPHP leadership on program initiatives in market
- Support education of senior health clinicians on high risk care related topics
- Provide virtual coverage for high risk care team patients as needed based on acuity and PTO needs
- Direct care for high risk patients through high risk and post-discharge visits (avg 1 day/ wk)
Qualifications:
- Must be a licensed physician (MD/DO) with an active license in at least one market (AZ/CO/WA). Willing and able to complete licensing in additional markets by start of role or within 3 months of being in role
- Board certification in Internal Medicine or Family Medicine. Additional board certification in Geriatrics or Palliative Care preferred
- Must reside within Phoenix, Denver, or Tacoma regions to be able to round with existing high risk care teams and interface with local practices. Willing to travel as needed to additional markets
- Minimum of 7 years experience with direct clinical practice, with preference for prior high risk, in home experience or other complex, value-based care settings
- Minimum of 4 years experience with progressive leadership
- Experience with team based care, excellent collaboration skills
- Deep experience with high risk, complex care
- Able to teach and model Geriatric and Palliative-focused care
- Availability during market time zones. Flexibility around work schedule
