JOB SUMMARY

Healthcare Navigators perform a wide range of tasks that are designed to promote engagement of individuals with substance use disorders and linkage with appropriate treatment services and supports. The goal is to improve access to treatment and recovery services which is enhanced by their own personal experience in dealing with mental illness and/or substance. To perform this job successfully, an individual must be able to perform each essential function satisfactorily. DUTIES AND RESPONSIBILITIES

  • Promotes the idea and concept of recovery by sharing their personal experience with mental illness/substance abuse and their own personal recovery experience within the aspects of a professional relationship.
  • Completes screening on consumers as needed to determine referral needs, then links to appropriate services including MAT, SUD residential, outpatient.
  • Completes required documentation for consumers involved in the program.
  • Assists in the treatment of consumers by providing paraprofessional support by helping consumers develop coping skills, self-help skills, and communication skills. Also, helps identify consumer’s individual warning signs and helps develop ways to prevent relapse.
  • Promotes recovery-based services and the value of the experience lived by individuals with mental illness or substance abuse.
  • Models and practices recovery principles in all activities including hope, empowerment, responsibility, and self-determination.
  • Assists with access to services including transportation if needed.
  • Completes all training and documentation within assigned timeframe/by policy.
  • Provides on-going follow-up with consumers referred to the program to promote recovery and support.
  • Assists consumers in developing natural supports and community living by learning multiple roles as friend, spouse, employee, student, etc., so a greater sense of identity is established.
  • Works with external customers to identify consumers that may benefit from treatment or support services.
  • Works as an active member of the collaborative team including being a part of internal and external team meetings such as service planning, MDT meetings, family team meetings, etc.
  • Maintains a working knowledge of Center policies, procedures, and licensure regulations, governing Prestera’s service delivery.
  • Maintains a working knowledge of available community resources and fosters a positive referral relationship with same to facilitate expedient treatmen
