JOB DESCRIPTION:
To provide comprehensive interdisciplinary care coordination for children and adults experiencing chronic life-limiting illnesses. Supports the coordination of services for children and their families with the goal of working toward better health outcomes and preventing unnecessary hospitalization by facilitating appropriate services. Provides education and training to family members and caregivers to best support and empower the knowledge and skills necessary to be self-directed and confident in caring for their child.
This position will also support our High Tech program.
EDUCATION:
• Current RN licensure or compact licensure recognized by the State of Vermont is required.
• Associate's degree in nursing required bachelor's degree preferred.
• Completion of a Pediatric Palliative Care Education Curriculum (within 1 year of hire).
• Completion of an approved Greif Counseling Course (such as Talk VT) within 1 year of hire.
• Current BLS and or ACLS Certification (must be maintained).
EXPERIENCE:
• Minimum two or more years working directly with the pediatric population and related home health/hospice or high-tech nursing experience
• Demonstrates self-reliance and the ability to make informed decisions while providing in-home care.
• Demonstrates cultural competence by respecting and understanding the diverse backgrounds and beliefs of patients and families.
