Job Summary
This
position is responsible for the oversight and provision of behavioral health
services and supervision of student interns in assigned locations. This
position will be expected to adhere to the NASW Code of Ethics and comply with
HIPPA regulations. Reports to the Principal Investigator.
Primary Duties and Responsibilities
- Manage operations of social work services within Forrest Health’s Family Medicine Residency Clinic (FMRC) to include required documentation and reporting to contract agency and appropriate leadership.
- Collaborate with patient
care team to identify and connect with patients in need of behavioral
health services and provide behavioral health consultation to medical
providers.
- Assess patient’s progress
and future needs, assist patients with adherence to medical treatment
plan, and coordinate referrals other community resources as needed.
- Provide ethical
behavioral health interventions, grief counseling, and support for substance
abuse issues to clinic patients.
- Utilize evidence-based
practices and clinical care management treatment strategies with patients
and their families.
- Provide clinical
supervision and management of assigned student intern schedule.
- Document all patient
contacts and communication in compliance with program protocols within the
electronic health record.
- Performs other duties as
assigned.
Minimum Qualifications
Licensure at the LMSW level. Master’s degree from
an accredited college or university in Social Work. At least one year of
experience in medical social work.
Knowledge, Skills & Abilities
Knowledge of integrated health practices and models of coordinated care.
- Demonstrated ability to provide supervision to student interns.
- Responsible for staying up to date on Continuing Education and professional license (LCSW).
- Responsible for obtaining and maintaining Professional Liability Insurance.
- Ability to work on an interdisciplinary team
- Demonstrate independent judgment in making patient care decisions based on field of expertise.
- Knowledge of behavioral medicine and evidence based treatments for medical and mental health conditions.
- Knowledge of common medical diagnoses and psychiatric disorders in child, adolescent and adult populations.
- Knowledge of growth and development of child/adolescent clients as related to all intellectual, emotional, spiritual, and physical aspects of their development.
- Knowledge of community resources.
- Knowledge of cultural diversity.
- Computer Skills: Ability to work with the following technologies: Proficiency with PC computer systems and MS Office Suite.
- Ability to travel across the U.S. Some overnights required.
