The Home Health / Hospice Quality Review Specialist II will support compliance with regulatory requirements related to documentation and billing, and ongoing clinical excellence through review of clinical documentation, joint visits with clinicians, and identification of educational needs. This position will perform chart reviews and home visits with clinicians, to identify opportunities for improvement, as well as areas of non-compliance with agency processes. Feedback will be provided directly to clinicians either 1:1 or through the provision of training classes. Trends identified will be communicated to the clinical management team, and the QRS II will participate in the development of quality improvement initiatives.

WHAT YOU CAN EXPECT
Reviews charts for pre-determined criteria as defined through the process or initiative, with timely completion of associated work queues.
Performs joint visits with clinicians to observe patient assessment skills and compliance with agency procedures.
Identifies clinical documentation deficits related to home health OASIS guidelines, reimbursement needs and compliance home health and hospice conditions of participation or CHAP standards.
Identifies opportunities for process changes and/or staff education related to documentation.
Coordinates with clinical management/education staff regarding staff educational needs and provides training and/or coaching with clinicians as directed.
Participates in performance improvement initiatives as directed.
Completes quality audits/special projects as directed.
Provide ad hoc guidance to clinical managers and clinicians on OASIS assessment timepoints and timing
QUALIFICATIONS
Associate's Degree- Nursing/Health Care Related- Required
Bachelor's Degree- Nursing/Patient Care- Preferred
1 year Home Health- Required
1 year Hospice- Preferred
3 years Home Health- Preferred
Registered Nurse (RN), or Physical Therapist (PT), or Occupational Therapist (OT) State Licensing Board- Required
