About the Role:
Are you a compassionate and skilled LPN looking to make a difference in patients' lives within the comfort of their homes? Holy Cross Home Care is seeking dedicated Home Care LPNs to join our growing team. You'll provide essential medical care and build meaningful connections with patients and families.

Key Responsibilities:

• Deliver high-quality nursing care to patients in their homes.

• Administer medications, treatments, and wound care as prescribed.

• Monitor patient conditions, recording vital signs and other important data.

• Educate patients and families about disease management and care plans.

• Collaborate with RNs, physicians, and other healthcare professionals.

Qualifications:

• 1-2 years of LPN Experience

• Active LPN license in the State of Maryland

• CPR certification.

• Previous home care or clinical experience preferred.

• Strong communication and organizational skills.

• Reliable transportation and willingness to travel within the service area.

The compensation range is from $25.54-40.87/hour and will be calculated at time of offer based on experience and internal equity.
