This position is full-time hybrid work schedule; 40 hours weekly, 8-hour shifts working Monday – Friday 8am-4:30pm.  No weekends or holidays.

As a Population Health Pharmacy Technician you will play a vital role in identifying, monitoring, and reporting on patients who have medication adherence gaps within the assigned value-based care contracts. Under the guidance of the Clinical Pharmacy Manager, this pharmacy technician collaborates with Population Health Clinical Pharmacists to analyze medication adherence reports, input intervention opportunities into a database, and verify patient medication adherence. Additionally, this position supports the CareVio transitional care pharmacist by conducting medication histories and managing prior authorizations.

PRINCIPAL DUTIES AND RESPONSIBILITIES:

  • Review medication adherence reports to filter out patients who do not qualify for intervention.
  • Input intervention opportunities into the EMR and Care Management program, prioritizing based on risk and assigning them to the appropriate pharmacist.
  • Perform patient outreach to address medication adherence gaps
  • Conduct follow-up calls, with pharmacies to confirm medication pickups
  • Assist patients in connecting with the pharmacist for various clinical needs
  • Facilitate processing prior authorizations and patient assistance applications for medications to expedite therapy initiation or avoid therapy disruption

EDUCATION AND EXPERIENCE REQUIREMENTS:

  • High school diploma or equivalent required.
  • Associate or bachelor’s degree preferred.
  • Experience working in a medical practice or pharmacy preferred.
  • Pharmacy Technician Certification Board (PTCB) certification preferred
  • Applicants with pharmacy experience, and not currently PTCB certified, are encouraged to apply and will be required PTCB certification in 1 year of employment.
