Lead Medical Case Manager

The Department of Infectious Diseases at the University of Virginia’s School of Medicine seeks applicants for a Lead Medical Case Manager. The Lead Medical Case Manager will work closely with clients living with HIV/AIDS who have multiple psychosocial and/or health related needs. Medical Case Management services is a strength- based approach to service that includes treatment adherence counseling, coordination and follow-up of medical treatments, client advocacy, and assistance in obtaining housing, financial support, legal services, social support and any other needed service. The goals of Medical Case Management are to insure that clients with HIV/AIDS have timely access to comprehensive medical care and social services; prevent disease transmission and delay of HIV progression and to promote and support client independence and self-sufficiency. The Lead Medical Case Manager will be supervised by the Medical Case Manager Supervisor .
The successful candidate will be expected to perform the following duties :
Lead Role
1. Works as a clinical resource to the case managers in the ID clinic to help address any patient care questions and urgent issues that need attention.
2. Training and onboarding the new case managers.
3. Collaborates with the Data Coordinator to run patient care reports and distributes to all case managers to use as they work with their patients daily.
4. Track all case manager assessments to make sure they are meeting their monthly goals.
Orientation and Assessment
1. Provide face-to-face assessment and re-assessment (including assessment of adherence to treatment)
2. Provide an overview of case management services including the role and responsibility of the case manager and the client.
3. Completion of the comprehensive assessment, within 30 days of initial screening, to determine the client’s strengths, resources, needs and problems.
4. Monitoring of HIV medication therapy to include education of client concerning risks and side effects, monitoring client adherence and tolerance of medications.
5. Document the CD4 count, results of viral load test, or prescribed ART medications every 180 days (every 6 months).
6. Client education about HIV, its transmission, complications, risk reduction and education.
7. Work with client to develop an individual Service Coordination Plan which includes realistic, measurable and mutually acceptable goals that are based on the results of the assessment.
8. Identify action steps needs to achieve each goal, including target dates for accomplishment of the stated goals.
9. Active linkages of client to appropriate agencies required to assist the client in achieving the goals and objectives identified in his/her Service Plan.
10. Obtain client and case manager’s signatures on Service Coordination Plan.
11. Copy plan and give to client and file original in client’s file.
Training and other Requirements :
The incumbent will be required to keep up-to-date on all required trainings of the grant/program.
The qualified candidate must possess:
Bachelor’s degree or equivalent experience in Human Service Field.
1 year experience working with persons living with HIV is preferred.
1-2 years Case Management experience preferred but not required.
PHYSICAL DEMANDS:
This is primarily a sedentary job involving extensive use of desktop computers. The job does occasionally require traveling some distance to attend meetings, and programs.
This position is located in Charlottesville, VA at UVA Medical Center, Infectious Disease Clinic
The anticipated start date is 02/28/24.
This position is a restricted position and is dependent upon project need, availability of funding and performance.
Anticipated Hiring Range: $50,000- 60,000 commensurate with education and experience.
