Your Specific Responsibilities:

This is a highly accountable position responsible for providing ongoing services to Healthy Start clients in Wakulla County under the direction of the Wakulla County Healthy Start Program Manager. The Healthy Start program is a home visiting program serving pregnant women, postpartum women, and infants from birth up until age three. This position will also provide services with Coordinated Intake and Referral (CI&R) as needed. Incumbents must meet requirements set forth by the Capital Area Health Start Coalition. Wakulla County Coordinated Intake and Referral (CI&R) program. The CI&R program provides telephonic services to pregnant women, postpartum women, and infants and their families where risks and needs are identified, and referrals are provided as needed. This position requires travel and is responsible for the integrity and security of data sets under his or her control. Uses knowledge of quality improvement (QI) methods and processes to work with other staff to help identify opportunities for improvement and develop a culture of quality within DOH Wakulla; assists in improvement projects, as assigned.

• Provide initial and ongoing Healthy Start services to Wakulla County Healthy Start clients using a home visiting model. Individualized services include providing pregnancy health education, parenting education, stress management support, nutrition education, breastfeeding education and support, smoking cessation, interconceptional care education, developmental screenings, screenings for other risks, and referrals to other agencies as needed.
• Provide initial intakes with clients referred to the CI&R program. Document all services in the designed client services database(s). Refer clients to alternative community resources appropriate to clients’ needs.
• Implement and coordinate parent support groups with focus of educating on newborn baby care, breastfeeding, and other prenatal and postnatal topics. Represent our agency and program area at community events and outreach activities to enhance community awareness and education. Develop relationships with community partners and resources within the community. Establish and maintain contact with community agencies.
• Attend Healthy Start meetings and monthly staff meetings as required by supervisor. Familiarize and follow the Connect Protocols and Healthy Start Standards and Guidelines.
• Other duties and responsibilities as assigned.

• Ability to establish and maintain relationships with clients while interpreting their needs.
• Ability to clearly communicate while interviewing.
• Ability to document in web-based health management system. 
• Ability to discuss sensitive maternal and child topics.
• Ability to follow contract and program guidelines.

Qualificactions:
Minimum - Valid Florida Driver's License Required, Knowledge and ability to discuss sensitive maternal and child topics with mothers and families, Ability to work before, during, and or/beyond normal work hours, including evenings and weekends as needed to meet with clients and participate in outreach activities. **Applicants must meet one or more of the following educational requirements:

a.) Minimum of a four-year college degree in social sciences; a health-related field such as nursing, health education, health planning, or health care administration; social work; or

b.) Associate degree and licensure as a Registered Nurse with three years of public health/maternal-child health experience or licensure as a Licensed Practical Nurse with four years of public health/maternal child health experience; or

c.)Two years of college with three years of public health/maternal-child health experience

Preferred - 2 or more years of experience professionally working with youth and/or families, experience teaching or educating individuals, bilingual in English and Spanish
