Our innovative Home & Community–based care model allows clients to receive integrated, functional support in the environments where recovery and reintegration truly happen.

We are seeking an experienced Licensed Professional Counselor (LPC) to join our team in Portland, OR in a flexible PRN W2 role (not 1099). This opportunity offers professional stability, clinical support, and the freedom to make a real-world impact.

What You’ll Do:

  • Provide counseling services that support clients’ emotional, cognitive, and behavioral adjustment throughout recovery.
  • Help individuals navigate life transitions related to injury or illness, including:
    • Regaining independence with daily responsibilities
    • Returning to work or school
    • Re-engaging in community and social activities
  • Collaborate closely with an interdisciplinary team, including Occupational Therapists, Physical Therapists, Speech-Language Pathologists, LCSWs, and other rehabilitation professionals.
  • Participate in team communication and care planning to support holistic, goal-driven outcomes.
  • Deliver services in real-world settings, allowing counseling to directly support functional progress.

Who We’re Looking For:

  • A licensed LPC in the state of Oregon with experience supporting individuals through recovery and life transitions.
  • A clinician who thrives in autonomous, community-based practice while valuing collaboration and communication.
  • Someone passionate about person-centered, functional care beyond traditional clinic walls.
  • Experience in neurorehabilitation, medical, or community-based settings preferred.

Responsibilities

  • Performs comprehensive evaluations, assessing the needs of patients and their families, and sets measurable objectives and goals that are formulated in conjunction with the rehabilitation team. Adjusts treatment plan to achieve maximum results in independence and/or function
  • Provides empirically based psychological interventions to the patients and families as appropriate for their specific needs to maximize and encourage their participation with the community at large
  • Designs community reintegration activities to assist the patient with their psychological/cognitive needs during physical activity
  • Assists the patient in understanding their needs and limitations and how to manage them during function
  • Regularly re-assesses effectiveness of treatment plan, progress of patient towards goals and the need for modifying goals or treatment
  • Communicates patient’s needs and progress to the treatment team, physician, pat
