JOB SUMMARY
Provide quality patient care to diverse patient populations using Medical Nutrition Therapy.
ESSENTIAL DUTIES AND RESPONSIBILITES
· Conducts nutritional assessment, proposes nutrition interventions, and provides follow-up to ensure nutritional needs of all patients and residents are met.
· This includes the full spectrum of care, including acute care, swing bed, long term care, ALF and outpatient service.
· Review therapeutic diet plans and menus to assure they are accurate and meet the special needs of the modified diet.
· Develop nutrition education materials based on current scientific evidence for hospital-wide use, including outpatient clinic.
· Provide necessary nutrition education to patient and family/caregiver as needed prior to discharge.
· Develop and execute educational and informative classes and events to promote better health and nutrition in Ferry County.
· Participate in the referral process for nutrition services (ie: referring inpatients for f/u as an outpatient; coordinating clinic MD referrals with schedulers in clinic).
· Collect and report on data for QI committee.
· Advise in-house staff of nutrition protocols and interventions.
· Field any complaints and grievances from personnel and patients in regards to special diets and/or diet restrictions.
· Participate in facility surveys as needed (inspections made by authorized government agencies).
· Coordinate with Dietary Manager to obtain special nutritional supplements and/or enteral feeding formula as needed
· Review and revise care plans and assessments as necessary, but at least quarterly (for LTC).
· Ensure that the care plan identifies any special equipment and utensils or any other special considerations the resident may need.
· Responsible for sharing the care plans with the Kitchen Manager.
· Maintains Registered Dietitian certification on a national level in addition to WA state licensure.
· Perform related duties as assigned by management.
PROFESSIONAL REQUIREMENTS
· Actively checks and responds to emails throughout the day.
· Completes all mandatory education within the given time-frame.
· Maintains regulatory requirements.
· Reports to work on time and as scheduled.
· Wears identification while on duty; uses computerized punch time system correctly.
· Represents the organization in a positive and professional manner in the community.
· Attends annual review and department in-services as scheduled.
· Attends at least 75% of staff meetings; reviews all monthly staff meeting minutes.
· Complies with all organizational policies regarding ethical business practices and the employee handbook.
· Communicates the mission, vision and values of the organization as well as the focus of the department.
· Meets dress code standards for the department; appearance is neat and clean.
· All Ferry County Health employees are required to maintain confidentiality in accordance with organizational policy, state and federal regulations, including not but limited to, the Health Insurance Portability and Accountability Act (HIPAA).
