Duties and Responsibilities:

  • Assess, diagnose, and treat patient’s symptoms and dysfunctional behaviors per the Diagnostic and Statistical Manual.
  • Provide psychosocial rehabilitation treatment, and individual and group psychotherapy for clients with anxiety, depression, chronic pain, hypertension, medication non-adherence, addiction, social and interpersonal problems, and challenging behaviors.
  • Develop individualized treatment plans in collaboration with other care team members and client.
  • Develop effective, trusting relationships with clients, with a focus on facilitating independence and maintenance of improved physical and mental health.
  • Assist clients with becoming involved in daily activities, socialization, health maintenance, sense of community, and progress in recovery reflective of the community model, which emphasizes choice, harm reduction, and psycho-social rehabilitation
  • Refer clients to agencies, support services, psychiatry, and other specialists as needed.
  • Provide mental health and case management services at various SCC sites as needed.
  • Provide functional assessments in reports to the Department of Public Social Services, Department of Child and Family Services, Social Security Administration, and the Judiciary.
  • Provide case management to assist patients with accessing vital resources such as housing, substance abuse treatment, specialty mental health care, and public benefits.
  • Document all assessments, diagnoses, care plans, interventions, and referrals in our electronic medical records system
  • Provide leadership and clinical consultation on mentally ill, homeless and supportive housing best practice models; generate recommendations for resources based on programming needs.
  • Provide consultation services to multidisciplinary team members by evaluating the biopsychosocial implications of the patient’s illness when the team is assessing and planning medical treatment. 
  • Consult with staff to ensure clients experiencing homelessness are connected to behavioral health services.
  • Work cooperatively with Behavioral Health Director to provide expertise and cultural sensitivity training to staff as needed.
  • In collaboration with Behavioral Health Director, outreach to community organizations for partnership opportunities with public, private, and nonprofit sector groups to provide services
  • Provide leadership and consultation to SCC staff in clinical crises with people experiencing homelessness

Position Requirements:

  • Masters’ degree in Social Work
  • Licensed as an LCSW
  • Minimum three years of providing mental health services (i.e., assessment, diagnosis, individual and group therapies) to homeless, mentally ill, substance addicted adult populations.
  • Prior experience working within a community mental health or primary care, medical setting preferred.
  • Able to work in a harm reduction model with clients who have multiple barriers.
  • Able to obtain and maintain CPR/1st Aid certification.
  • Two years’ post licensure work experience preferred
  • Knowledge and experience in chemical dependency and substance abuse treatment preferred
  • Bilingual skills in languages such as Spanish, Korean, Tagalog, Russian, Armenian preferred.
