Physical Therapist - Home Health, Bandera, Hondo area

Touchstone Health, a full-service home health agency, is seeking a qualified and compassionate Physical Therapist to care for our patients. Under the supervision of the Clinical Manager and in accordance with the interdisciplinary plan of care, our therapists evaluate, establish, and implement individual programs of care for our patients to increase functional ability and reestablish independence at home. Travel to San Antonio and surrounding area. (Far Northwest and Bandera area)

Duties & Responsibilities:

• Performing home visits to evaluate patient's level of function
• Developing an individual plan of care based on the client's personal goals
• Implementing the treatment program through direct treatment of the patient
• Reassessing clinical signs and symptoms to determine effectiveness of program
• Instructing patient, family, and staff regarding participation in the treatment program
• Collaborating with PTA and additional interdisciplinary staff member to improve treatment program

Requirements

Qualifications & Requirements:

• Degree in physical therapy from a university accredited by the American Physical Therapy Association
• License to Practice by the Texas
• Comprehensive knowledge of the principles, concepts, and methodology required of an physical therapist and its practice application in the treatment of home health patients at home
• Excellent Communication and documentation skills
• Willingness to work with a interdisciplinary team
• Valid Texas driver’s license and reliable, insured transportation
