The ACN and DCPH is comprised of 14 primary care sites, seven school-based health centers, 13 mental health school-based programs, and over 60 specialty practices. It also oversees over 20 community programs, including a NYS Health Home Program. The Comprehensive Health Program (CHP), the site of this position, is a center of excellence for HIV treatment and prevention as well as sexually transmitted infections and general infectious diseases. To address social determinants of health and mitigate risk for poor outcomes among vulnerable populations, CHP has embraced comprehensive care coordination strategies and practices multidisciplinary, team-based care including integrated behavioral health services, intense care coordination and primary care. Patients are cared for by a team of multidisciplinary providers (care coordinators, SW, RN and primary care provider) and are offered Health Home enrolment as part of this comprehensive care. NYS’ Health Home programs are designed to address complex and high-cost medical conditions and to address medical, behavioral and social needs comprehensively. Care teams work collaboratively to develop patient-centered care plans that empower patients to be an active participant in his/her health care.
Program Manager – Comprehensive Health/Health Home & Care Coordination - Grant Funded
We seek a program manager to oversee our care coordination programming and Health Home program activities. The Program Manager will lead the care coordination and Health Home programs along with another Program Manager, overseeing all day-to-day operations and monitoring all program requirements (billing, documentation, data reports). The Program Manager will guide implementation of the strategic vision of the program in order to ensure its sustainability and longevity. The Program Manager will collaborate with team members and specifically will oversee all team care coordinators by providing supervision, coaching, guidance and feedback towards the shared aim and vision. The ideal candidate will need strong organizational skills and be able to interpret and apply findings from data reports to ensure high fidelity of data for programmatic requirements, documentation and billing. Additionally, the Program Manager ideally would have familiarity with Grant Funded program implementation, evaluation, and management as the Program Manager will participate with the team to help continue implementation and enhancement of existing grant funded programming which the care coordination staff participate in. At the same time it is crucial for the Program Manager to be able to effectively supervise care coordinators and possess an excellent ability to communicate verbally and through writing. Case management and/or Behavioral Health experience will allow the Program Manager to guide care coordinators to resolve clinical and administrative challenges. Program Managers will participate in the program’s Care Coordination Steering committee meetings and collaborate on future directions of the program. Relationship building and networking skills are crucial to this role, as the Program Manager will collaborate with many NYP departments and community organizations to ensure the ongoing success of the program and increase its internal and external visibility. The Program Manager will closely track program outcomes to routinely report back to various groups, including funders, senior leadership, and other stakeholders.
Preferred Criteria
Bilingual English/Spanish
Master’s in social work/related clinical field or Master’s in Public Health
Experience in program management, development, implementation and evaluation
Interest in Quality Improvement strategies/planning
Familiarity with care coordination and program management including ensuring deliverables are met and reporting out to key stakeholders.
Comfort with data reports and tasking team members to achieve high quality outcomes for patients and meet programmatic requirements.
