Job Description

Decentralized and centralized pharmacist activities supporting the 425-licensed bed hospital.  Activities may include supporting the Emergency Department which averages 180+ visits per day and hospitalist provider for ED admissions, the observation unit, the 24-bed Intensive Care Unit, Cardiovascular Intensive Care Unit or general medicine units with some centralized activities.    

Decentralized activities approximate 60% of time. 

The Clinical Pharmacist reflects the mission, vision, and values of NMHC, adheres to the organizations Code of Ethics and Corporate Compliance Program, and complies with all relevant policies, procedures, guidelines and all other regulatory and accreditation standards.

Under minimal supervision, performs clinical and educational services of the Pharmacy Services Department. Disseminates pharmaceutical information to the medical staff and hospital departments. Functions as a staff pharmacist as required.

Responsibilities:

Conducts pharmacotherapeutic dosing services, such as pharmacokinetic, hyperalimentation, and anticoagulation. Includes review of patient chart, analysis of drug therapy prescribed by the physician, and check for drug interactions, incompatibilities, and appropriateness of therapy. Takes into consideration the special needs, such as age or renal function, of the patient when determining initial dose and dose adjustments.

Completes the following activities for patients identified by Pharmacy Committee approved criteria, drug utilization evaluation activity, or physician order:

a.     Adjusts or recommends adjustment of drug therapy according to changes in concomitant therapy or patient condition.

b.     Identifies potentially significant drug-drug, drug-food, drug-laboratory, and drug-disease interactions and makes appropriate recommendations.

c.     Recommends drug discontinuation or dosage alteration when indicated.

d.     Recommends chugs that are appropriate and cost effective.

Obtains, records and utilizes clinical laboratory data to evaluate appropriateness of drug selection and/or dosing regimen.

Documents all activities appropriately.

Information Retrieval

Serves as a resource person for staff pharmacists, nursing staff, medical staff and other hospital departments.

Researches and prepares special reports and projects as requested by the Director of Pham-lacy, Assistant Director of Pharmacy or Pharmacy Clinical Coordinator.

Evaluation of Drug Usage and Therapy

Monitors and participates in hospital protocols utilizing pharmaceuticals.

Participates in the adverse drug event program including; detecting events, completion of hospital adverse drug reaction forms, follow-up and written reports.

Participates in the drug utilization review activities of the department.

Participates in multidisciplinary rounds, where applicable, consults with medical personnel, answers questions and makes suggestions concerning optimum patient care with respect to drug therapy.

Actively participates in the departmental Quality Improvement program.

Utilizes drug usage evaluation information to effectively alter drug therapy.

Education

Conducts inpatient/outpatient medication counseling and patient education.

Conducts blood glucose monitoring instruction.

Assists in the precepting of Pharmacy students and PGYl or PGY2 residents.

Prepares materials for use on educational programs and related publications.

Conducts pharmaceutical education programs for the medical staff, nursing staff, pharmacy staff and other ancillary departments.

Continually updates and upgrades knowledge database on new therapeutic concepts and new drugs.

Miscellaneous and Personal

Practices safe work habits in compliance with the hospital 's exposure control plan.

Functions as a Staff Pharmacist as required (see job description 415.007)

Performs other inherent duties, related, but not limited to the above.

This position is full-time, 8-hour shifts, day shift, 7:00 a.m. to 3:30 p.m., during the week with every 3rd weekend (Saturday and Sunday).
