Purpose: (General description and summary)
The Mental Health Clinician (MHC) provides clinical and supportive services to participants within the Rosebud program as part of the multi-disciplinary team. Rosebud is a supportive housing program providing both clinical services through Medicaid and wraparound supports through a contract with DHS to participating youth ages 16 to 18 who are in the foster care system. The position is responsible for assessing mental health symptoms, providing diagnoses, and developing, implementing, and monitoring individualized treatment plans specifying medically necessary services for treatment and recovery. The position supports in the provision of skills training, case management, counseling, and all other applicable services to individuals. Interventions take place in , home, and community settings. There are significant administrative responsibilities in terms of documentation, reauthorization, and other regularly occurring events.
Accountabilities:
Case Planning and Management
- Develop relationships with each client on the caseload by meeting for case management, skills training, and/or counseling on an established schedule.
- Assess the strengths and service needs of each participant.
- Develop individual service plans in partnership with the participant and chosen supports.
- Collaborate with DHS caseworkers to create plan for family involvement and integration of family support.
- Provide clinical recommendations related to treatment, crisis intervention, and service needs to Rosebud team and serve as a point of consultation on clinical issues.
- Assist participant in connecting to resources and services needed to achieve goals set in individual service and support plan.
- Monitor service delivery and coordinate care as needed.
- Develop relationships with providers and stakeholders to facilitate service provision.
- Ensure scheduling and attendance of prescriber visits for medication management and problem-solve with participants to ensure effective medical services.
- Provide customer service to New Narrative, Inc. stakeholders.
Counseling
- Provide effective behavioral health counseling and psychotherapy to participants in both individual and group settings.
- Regularly assess and monitor therapeutic progress, ensure that counseling services provided remain medically necessary and consistent with the mental health assessment and individual service plan.
- Provide limited-duration family style therapy that aligns with participant’s individual goals and provide referrals to long-term family therapy, as needed.
- Utilize and demonstrate proficiency with Medicaid-approved, evidence-based practices
- E.g. motivational interviewing, cognitive behavioral therapy, dialectical behavior therapy, wellness management and recovery, integrated dual diagnosis treatment, etc
Crisis Intervention
- Collaborate with Compliance Specialist to review incoming safety plans and ensure use of safety plan in ongoing services.
- Take reasonable action to avoid use of crisis services, including consistent use of trauma informed care principles. Collaborate with participant and chosen support on any other proactive steps to minimize us
