The Population Health Pharmacist embedded in Downtown Health Plaza Clinic will be accountable for carrying out medication-related strategies designed to optimize patient care and outcomes in the WFBH value-based care (VBC) population specific to Managed Medicaid. These strategies seek to improve health and reduce the total cost of care while considering social determinants of health. The pharmacist in this role will: provide patient-centered care to Managed Medicaid attributed to the health system; aid in the closure of medication-related care gaps through completion of comprehensive and targeted medications reviews, face-to-face clinic visits, home visits, and outreach calls; ensure access to medications; triage appropriate utilization of pharmacy resources; precept residents (PGY1 and PGY2) and students; conduct medication utilization evaluations; participate in population health research projects related to Managed Medicaid; participate on Medical Center and Department committees. In addition, the pharmacist will be expected to build relationships, facilitate teamwork, and serve as a high-performing team member; coordinate and participate in the education of pharmacists and other health care professionals regarding Managed Medicaid topics; assist in the design and implementation of services specific to the Managed Medicaid population.
The Pharmacist will work collaboratively with members of the multidisciplinary population health team, clinic staff, Managed Medicaid case managers, retail and specialty pharmacies, and other areas as needed. This individual must have strong problem-solving, emotional intelligence, creativity, and written and verbal communication. This individual must also be able to quickly adapt to change and maintain a positive attitude. Education/Experience/Certifications to Qualify:
- Graduate of an accredited School of Pharmacy with a B.S. in Pharmacy or Doctor of Pharmacy (Pharm.D.) is required.
- Current state pharmacist license in good standing through North Carolina Board of Pharmacy is required.
- CPP may be required (TBD)
- Completion of an ASHP-accredited PGY1 residency or equivalent experience is required.
- PGY-2 residency in ambulatory care or managed care preferred.
- Board Certification preferred.
- Managed care experience preferred; community, specialty or ambulatory care experience preferred.
Major Responsibilities:
- Oversees pharmaceutical care, including retrieving and evaluating biomedical literature and designing medication therapy regimens.
- Uses effective patient education techniques to provide counseling to patients and caregivers, including information on medication therapy, adverse effects, compliance, appropriate use, handling, and medication administration.
- Designs, recommends, monitors, and evaluates patient specific pharmacotherapeutic regimens based on relevant biomedical literature. Implements recommendations effectively and utilizes policies and procedures and documents in the appropriate format.
- Reviews and accurately interprets medication orders and prescriptions
- Participates in the management of medical emergencies when applicable
- Actively supports department medication safety and quality improvement activities, including the reporting of events through appropriate channels.
- Performs quality assurance and performance improvement of the medication use system as appropriate.
- Collaborates with the healthcare team to manage formulary adherence and the medication-use process. Assists in the acquisition of non-formulary, compassionate use, and investigational medications.
