Overview
Dartmouth Health is seeking a dedicated Nurse Practitioner (NP) or Physician Assistant (PA) to join our Outpatient Stroke Program, providing high-quality, patient-centered follow-up care for individuals recovering from stroke and other cerebrovascular conditions.
This role offers the opportunity to work alongside stroke neurologists and a multidisciplinary team to improve outcomes, prevent recurrence, and support patients in regaining their quality of life.

What You’ll Do:
Conduct post-stroke follow-up visits, review hospital discharge plans, and monitor recovery progress
Manage secondary stroke prevention strategies, including medication management and lifestyle counseling
Order and interpret imaging, lab work, and other diagnostics as needed
Educate patients and families on stroke risk reduction and symptom recognition
Coordinate care with primary care providers, rehabilitation services, and other specialists
Document patient encounters accurately and thoroughly in the EMR
Qualifications:
Ability to obtain/maintain licensure as an NP or PA in New Hampshire
National certification as an NP or PA
- At least two years of independent clinical experience
Neurology, vascular, or primary care experience preferred (stroke experience a plus)
Strong communication, organization, and patient education skills
