Position Overview Flex Rehab Solutions is actively seeking a Licensed Physical Therapist to join our dedicated team in Port Elizabeth, New Jersey. This full-time role will primarily focus on delivering exceptional patient care in a home care setting. Work Environment This role is based onsite, delivering personalized care directly in patients' homes, where you will have the opportunity to witness and assess their progress in real-time. Key Responsibilities Conduct comprehensive evaluations to determine patients’ physical capabilities, limitations, and rehabilitation goals. Develop and implement individualized treatment plans focused on improving mobility, strength, endurance, and functional independence. Guide patients through therapeutic exercises and activities, providing hands-on assistance and education about proper body mechanics. Utilize various therapeutic modalities, such as electrical stimulation, ultrasound therapy, and heat/cold therapy to enhance treatment outcomes. Monitor and document patient progress diligently, updating treatment goals and strategies based on their evolving needs. Maintain a clean and organized treatment area, ensuring that all necessary equipment and supplies are readily available for sessions. Collaborate with a multidisciplinary team, including occupational and speech therapists, to optimize patient care and ensure comprehensive treatment plans. Provide emotional support and motivation for patients throughout their rehabilitation journey, fostering a nurturing environment conducive to healing. Adhere to ethical practices and ensure that patient confidentiality and rights are upheld at all times. Patient Population / Client Focus As a Physical Therapist at Flex Rehab Solutions, you will work with a diverse patient population, including individuals recovering from surgeries, athletes rehabilitating from sports injuries, children requiring pediatric therapies, and seniors focused on maintaining independence. By working in home care settings, you can adapt therapy to the patient's daily living environment, enhancing the effectiveness of treatment. Qualifications Bachelor's degree in Physical Therapy from an accredited institution. Active Physical Therapist license in the state of New Jersey. Minimum of 1 year of clinical experience in physical therapy, preferably in a home care setting. Strong interpersonal and communication skills to effectively collaborate with patients and multidisciplinary teams. Demonstrated ability to motivate and support patients through their rehabilitation processes. Preferred Experience Experience with diverse populations, including geriatric and pediatric patients. Familiarity with evidence-based practices and documentation protocols. Certification or training in specialized therapeutic techniques, such as myofascial release, dry needling, or manual therapy, is a plus. Compensation and Benefits Flex Rehab Solutions offers a competitive annual salary ranging from $100,000 to $120,000, commensurate with experience.
