$76,398.00 - $99,320.00The referenced pay range represents the minimum and maximum compensation for this job. Responsibilities:
GENERAL PURPOSE:Provides individualized assistance, support and comprehensive coordination of care for patients and their families/caregivers, to eliminate barriers to timely care, facilitate flow through system, facilitate interactions with care team, increase patient and provider satisfaction, maintain point of contact with providers and serve as first point of contact for patients and families.

UR Medicine Orthopaedics is offering an exciting opportunity to work with the UR Ambulatory Hand Division team at AO Marketplace (Henrietta) location. The Nurse Navigator will play a crucial role in guiding patients through the entire continuum of care related to hand, wrist and elbow. The Navigator is a key member of the multidisciplinary team helping to remove barriers for patients and provide resources for pre and post-surgical treatment. This role ensures defined standards at every episode of care for screening, assessment, planning, patient education, managing and communicating to assure the progression of safe and efficient care throughout the continuum in a collaborative approach. Responsibilities include coordination of care, telephone triage including ED, hospital follow-up, and urgent hand referrals, Mychart messaging, patient education, and wound care. Must possess strong assessment, communication and computer skills, be proactive and independent in decision-making and have good organizational skills.
