The Care Manager, Telephonic Nurse 2 , in a telephonic environment, assesses and evaluates members' needs and requirements to achieve and/or maintain optimal wellness state by guiding members/families toward and facilitate interaction with resources appropriate for the care and wellbeing of members. The Care Manager, Telephonic Nurse 2 work assignments are varied and frequently require interpretation and independent determination of the appropriate courses of action.
The Care Manager, Telephonic Nurse employs a variety of strategies, approaches and techniques to manage a member's physical, environmental and psycho-social health issues. Occasional travel to the office in SC (approx 5%) as needed to meet with business groups.
This position will specifically focus on Medicaid pediatric and maternity members.
- Identifies and resolves barriers that hinder effective care.
- Ensures patient is progressing towards desired outcomes by continuously monitoring patient care through assessments and/or evaluations.
- May create member care plans.
- Understands department, segment, and organizational strategy and operating objectives, including their linkages to related areas.
- Makes decisions regarding own work methods, occasionally in ambiguous situations, and requires minimal direction and receives guidance where needed.
- Follows established guidelines/procedures.

Required Qualifications
- Licensed Registered Nurse (RN) in the state of South Carolina with no active disciplinary action
- 3+ years combined clinical Obstetric/Pediatric experience
- 1 year pf Case Management experience
- Comprehensive knowledge of Microsoft Office applications including Word, Excel, and Outlook
- Must reside in South Carolina and be able to work Monday - Friday, 8am - 5pm EST.
Preferred Qualifications
- Bachelor's degree
- Experience with discharge planning and patient education for adult acute care
- Medicaid Experience
- Managed care experience
- Certified Case Manager (CCM)

- Associates who live and work from Home in the state of California, Illinois, Montana, or South Dakota will be provided a bi-weekly payment for their internet expense.
- Humana will provide Home or Hybrid Home/Office associates with telephone equipment appropriate to meet the business requirements for their position/job.
- Work from a dedicated space lacking ongoing interruptions to protect member PHI / HIPAA information
