Private Duty LPN - Day Shift

 

Location: Kenton, TN

 

Schedule: 6 AM - 6 PM, Every Other Weekend | ¾ Split Schedule (36 hours one week, 48 hours the next)

We are currently seeking a dedicated LPN to join our Private Duty team in Kenton, TN. This role focuses on providing exceptional care for a patient with a G-tube, ensuring their comfort and safety in the home setting.

 

Role Highlights:

• Shifts: 6 AM - 6 PM with a ¾ Split Schedule
• Week 1: 36 Hours
• Week 2: 48 Hours
• Team Setup: 2 nurses covering day shifts on a rotational basis

Requirements:

• Current LPN license in TN
• Experience with G-tube care is required
• Ability to work every other weekend

Responsibilities

• Perform selected nursing procedures within the scope of LPN practice
• Provide patient and family teaching according to plan outlined by the RN
• Monitor patient's vital signs and make specific measurements and observations
• Monitor and record intake and output. Report significant changes immediately
• Observe, assess and record patient's physical and mental condition and reports any changes
• Administer medications as instructed and as ordered by the physician and instructed by the RN and document the patient's file accordingly
• Attend and participate in multi-disciplinary patient case conferences, staff meetings and in-service education programs as assigned

Qualifications

• Licensed Practical Nurse / LPN license in good standing
• Previous private duty, home health, hospice, hospital or skilled nursing experience preferred
• Active CPR certification
• Familiarity with electronic health records
• Strong problem solving and critical thinking skills
• Reliable transportation, valid driver's license and current automobile insurance
