Position Summary
• The Post Acute Care nurse provides office support for all Post-Acute Care Providers as a liaison between providers, facilities, patients, and families.
Education
• Associates Degree in Nursing
Licenses and Certification
• Current Illinois Registered Nurse License
• BLS Certification
Experience and Skills
• EPIC and a working knowledge of Post Acute Care facility EMRs
Role Specific Responsibilities
- Assess and plan patient care needs
- Assist provider in Post Acute Care office setting
- Place order s and referrals and follow up
- Field and triage incoming patient and family calls
- Answer time-sensitive fax
- Be point of contact for distributing lab results and orders to facility nurses
- Some travel required
