The Clinical Documentation Specialist (CDS) supports all care teams across the region in comprehensive and targeted medical chart reviews -- to ultimately facilitate efficient and effective medical care for our patients. CDSs are trained to be experts in structured clinical assessments, accurate and specific documentation, population health workflows, and team based care.
Core Responsibilities:
Completes in-depth patient chart reviews of both inpatient and outpatient medical records to ensure accurate representation of severity of illness and complexity of care
Indicates documentation not yet included in the record, but indicated by clinical factors concurrently contained in the record, to improve both accurate and specific documentation and patient care
Identifies and analyzes opportunities for improving patient quality outcomes and cost of care
Analyzes chronic condition prevalence based on patient demographics and identify opportunities for interventions

What we're looking for:
In-depth knowledge of medical coding procedures for the Medicare-eligible population, including ICD-10 and HCC codes
In-depth knowledge of disease processes as well as medical and surgical specialities
Ability to drive accuracy and specificity of clinical documentation and diagnostic data
Ease and expertise working with multiple technical systems, including Excel and EMRs
Ability to collaborate across multiple teams and engage in multiple perspectives to work toward a common goal
Proven experience and ability to build relationships with Providers
Proficiency in tracking down and leveraging external resources, such as hospital records, Blue Button, and payer data, to support retrospective and prospective accurate documentation
Ability to succeed in a fast-paced, ever-changing environment
Self-starter with a high degree of drive, initiative, and follow through
Ability to develop, plan, and implement short and long term goals
Exceptional interpersonal, written and verbal communication skills
Commitment to company vision
Experience in accurate and specific documentation
U.S. work authorization

Registered Health Information Technician (RHIT)
Certified Coding Specialist (CCS)
Certified Risk Coder (CRC)
