Caseworker- Senior (State Hospital Step-Down Program)

The Senior Caseworker will provide coordination and intensive community-based services for individuals transitioning from state psychiatric hospitals into supervised residential settings. As part of the State Hospital Step-Down Program, this role emphasizes community reintegration, housing stability, care planning, and support for individuals with serious mental illness (SMI), including co-occurring substance use disorders.

•    Assist in conducting comprehensive psychosocial assessments and developing individualized service and recovery plans in coordination with a multidisciplinary team.
•    Assist clients in securing stable housing, identifying community resources, and navigating Medicaid and other social service systems.
•    Provide support with coordination of mental and physical health care, substance use treatment, benefits, and transportation.
•    Maintain regular contact with residents in their homes to monitor progress, provide psychosocial treatment, and adjust care plans as needed.
•    Participate in discharge planning, pre-placement interviews, and collaborative meetings with hospital staff and residential providers.
•    Serve as a liaison with community providers, landlords, family members, and legal systems as appropriate.
•    Ensure accurate documentation and timely reporting of services provided.
 

• Bachelor’s degree in Social Work, Psychology, Sociology, or a related field.
• Minimum of three (3) years of related experience providing caseworker services in a mental health, housing, or community support setting.
• Demonstrated knowledge of SMI, trauma-informed care, recovery-oriented practices, and housing navigation.
• Strong organizational, communication, and problem-solving skills.
• Ability to work both independently and as part of a collaborative care team.
• Familiarity with supervised housing programs, Medicaid service coordination, and psychosocial rehabilitation is a plus.

Additional Information

This position requires travel between the hospital, residences, and community agencies. Travel reimbursement is available based on state allotment guidelines
