TERRITORIAL EXPANSION OPPORTUNITY
This is a unique opportunity to join our Eastern Ohio expansion into Ashtabula, Columbiana, Mahoning, and Trumbull counties. As we launch services in this new territory, we are seeking an LPN to help establish our presence and grow with us.
Position Overview
This is a per-visit role. It is not an hourly job. You are paid for each completed visit or task: $25 per supervisory visit, $35 per open visit, $35 per skills test, and $35 per hour of office time for written tests, plus mileage reimbursed at $0.655 per mile.
IMPORTANT: This is an expansion role perfect for supplemental income, not primary employment. Initial hours will be minimal (potentially 5-10 hours per week or less) as we build our client base in Eastern Ohio from the ground up.
Here is what that looks like at the start. This is a brand-new territory, so we begin with no clients. We may bring on about one new client a month, and some months none at all. Each new client means one open visit. After that, every active client gets a supervisory visit every 60 days. So the work builds as the client base builds: the more active clients we have, the more supervisory visits there are, and the more you earn. In the early months that is a small number of visits per month.
The Licensed Practical Nurse in this supervisory role is responsible for completing open visits, collecting documentation, conducting supervisory visits to check on Home Health Aides every 60 days, and completing pre-employment testing for aide candidates. This position does not involve skilled nursing care or medical assessments.

Key Responsibilities
- Care Plan Implementation: Assist in carrying out personalized care plans as directed.
- Documentation: Prepare accurate and thorough documentation according to agency standards and regulatory requirements.
- Supervisory/Open Visits: Conduct regular supervisory visits every 60 days to check on Home Health Aides and complete open visits for new clients.
- Pre-Employment Testing: Conduct skills tests and written tests for Home Health Aide candidates.
- Team Collaboration: Foster cooperative relationships among the healthcare team by understanding each person's role and actively participating in case conferences.
- Professional Development: Assume responsibility for ongoing professional growth through participation in nursing organizations and in-service programs.
Experience
- One or more years in a home health agency or in a hospital setting. Home health experience preferred.
- Experience with scheduling, documentation, and compliance regulations not required but a plus.
