Position Overview

United Health Services (UHS) is seeking a Transportation Coordinator to manage and optimize the movement of patients, equipment, and supplies throughout our healthcare system. In this role, you will create and adjust transportation schedules, dispatch staff, track service requests, and ensure compliance with safety protocols and healthcare regulations.

Job Responsibilities

  • Coordinate patient transportation services to ensure timely and safe travel to and from medical appointments within the UHS System.
  • Maintain accurate and timely documentation of all transportation arrangements and communications in the EPIC electronic health record system.
  •  Communicate with patients, caregivers, and transportation vendors to confirm scheduling details and address specific mobility or support needs.
  •  Monitor and assess the effectiveness of current transportation workflows and processes; recommend improvements to increase efficiency and patient satisfaction.
  • Collaborate with care team members, including nurses, social workers, and case managers, to align transportation planning with overall care coordination.
  • Attend interdisciplinary care conferences as needed to provide transportation input and help identify solutions to access-related barriers.
  • Respond promptly to patient and provider concerns or complaints related to transportation, and escalate issues to the supervisor when appropriate.
  • Stay informed of transportation resources, eligibility criteria, and coverage options available to patients based on insurance, location, and medical necessity.
  • Assist in executing non-clinical tasks assigned as part of Population Health Care Pathways, supporting patient outreach, follow-up, and navigation.
  •  Track transportation trends and service gaps that impact appointment attendance, reporting findings to leadership for potential program development.
  • Serve as a liaison between UHS departments and external transportation providers to maintain reliable service partnerships and address operational challenges.
  • Support departmental goals by participating in performance improvement initiatives and staff training activities related to patient transportation services.

Position Qualifications

Minimum Required:

  • High School Diploma or equivalent.
  • Two (2) years of experience collaborating as part of a care team to coordinate community resources to address social determinants of health.

Preferred:

  • Customer Service experience.
  • Previous experience with direct patient care.
