Description

Hospice of the Piedmont is seeking a full-time RN Case Manager - Community Care Partner in the Charlottesville and surrounding counties to manage cases assigned and provide consistent, quality patient care.  Essential duties include, but are not limited to:  

  • Performs nursing evaluation visits to new admissions and assists in determining eligibility based upon agency criteria, assuring compliance with all Federal, State, and Hospice regulations and policies.
  • Collaborates with interdisciplinary team to develop the Plan of Care and Nursing Care Plans in conjunction with physician’s orders.
  • Regularly re-evaluates patient needs, consulting with patient’s physician, reporting changes which might necessitate a change in the Plan of Care and communicates this with Hospice employees and contractors involved in the patient’s care.
  • Practices appropriate documentation at bedside using EMR and prepares clinical progress notes on nursing services delivered in a timely manner.
  • Assists supervisor in conducting clinical observation visits of LPN’s and HHA’s and in preparing written performance evaluations.
  • Provides guidance and direction to employees reporting to him/her. Ensures staff receives needed training, and reviews and adjusts staff work duties as required.
  • Proactively supports the Individual Achievement Review and Evaluation (IAR &E) performance evaluation process. 
  • Assumes responsibility for care given by Hospice Home Aide assigned through clear instruction and bi-weekly on-site reviews.
  • Participates in taking on-call responsibilities on weekends/holidays on a rotating basis with other staff nurses as needed.
  • Follows company personnel policies and procedures (i.e., recruitment and hiring process, completion of accident reports, etc.) 
  • Attends required meetings and participated in committees as requested.
  • Maintains patient confidentiality.

Typical Physical Demands:

  • The physical demands may require employees to regularly use their hands, sit, stand, stoop, bend, reach, lift, walk, and climb steps. Specific vision abilities required include close vision, distance vision, color vision, peripheral vision, depth vision, and the ability to adjust focus.
  • Lifting up to 20 pounds, push up to 200 pounds in a wheelchair.  
  • Frequent travel may be required.

Requirements

  • Graduate of an accredited nursing school.
  • Licensed as a Registered Nurse in the Commonwealth of Virginia.
  • Two years of relevant nursing experience.
  • Good oral and written communication skills.
  • Must demonstrate basic computer skills.
