Clinical Quality MA/CMA

Job Summary:

A member of the clinical quality team, the Clinical Quality Certified Medical Assistant will work with members to close quality gaps in care for all Good Samaritan Physician Network sites. The role our Clinical Quality CMA provides is to participate in ambulatory quality reporting to help close gaps in patient care.  This role will provide patient outreach to patients followed by supplemental data submission to various value based contract portals.

Essential Job Duties:

1. Attend and actively participate in all Value Based Contract meetings, trainings & patient outreach
2. Schedule patients for Medicare Annual Wellness Visits
3. Update patient chart prior to Medicare Annual Wellness Visit
4. Enter supplemental data into EPIC/insurance portals to close gaps in care
5. Make referrals per established protocols and assist patient in coordinating preventative testing follow up
6. Assist Transitional Care Management
7. Assist with One Call Discharge

Secondary Job Duties That May be Reassigned:

1. Performs other duties as assigned or requested

Job Specifications:

• Education:  Graduate from an accredited Medical Assistant program required.
