HealthPartners is hiring a MTM Manager. They will provide management and oversight of the medication therapy management (MTM) program for the health plan and HealthPartners clinics to optimize medication use and support the Triple Aim of quality, experience, and affordability. This role provides management and supervision of the delivery of medication therapy management at HealthPartners clinics and to oversee the plan MTM provider network overall. They assist in providing guidance and support for clinical pharmacy programs including medication therapy management in coordination with other pharmacy and clinical leaders across the HealthPartners enterprise. This role also serves as the Residency Program Director for the HealthPartners PGY1 Managed Care Residency Program.
This position manages a team whose accountabilities support the development and management of the MTM program.
MINIMUM QUALIFICATIONS:
- Education, Experience or Equivalent Combination:
- Bachelor’s degree or Pharm. D. from an ACPE accredited school of pharmacy
- Five (5) years of clinical pharmacy experience preferably in managed care or ambulatory care OR
- Three (3) years of clinical pharmacy experience if completed an ASHP-accredited PGY1 residency OR
- 1 year of clinical pharmacy experience if completed an ASHP-accredited PGY2 residency.
- History of serving as a preceptor within a residency program and/or advanced/introductory pharmacy practice experiences (APPE/IPPE).
- Licensure/ Registration/ Certification:
- Licensure in the State of Minnesota as a pharmacist.
- Licensed preceptor in the State of Minnesota.
- Knowledge, Skills, and Abilities:
- Knowledge of and experience providing comprehensive medication management services.
- Knowledge of managed care pharmacy practice.
- Knowledge of pharmacology, pharmacodynamics, and pharmacokinetics.
- Knowledge of pathophysiology and therapeutic disease states commonly found in ambulatory patients.
- Excellent verbal and written communication skills.
- Computer and software literate.
- Ability to implement changes and develop innovative roles.
- Interpersonal and communication skills necessary to obtain cooperation among Pharmacy, Nursing, and Medical Department.
PREFERRED QUALIFICATIONS:
- Education, Experience or Equivalent Combination:
- Previous supervisor/management experience.
- Completion of a pharmacy residency (managed care or ambulatory care preferred).
- Experience in administering Medicare PartD compliant MTM programs.
- Served as a residency program preceptor and/or residency program director.
- Licensure/ Registration/ Certification:
- BPS certification
- Knowledge, Skills, and Abilities:
- Ability to interpret data and generate reports.
ESSENTIAL DUTIES:
- (50%) – Executes MTM strategy at the health plan and in HealthPartners clinics.
- Participates in the development of the MTM program strategic plan.
- Tracks ongoing performance of the MTM team towards meeting their operational goals.
- T racks ongoing performance of the contracted MTM network to ensure the health plan is meeting operational and regulatory goals and requirements.
- Ensures MTM programs are operating in compliance with applicable federal and state laws and regulations.
- Oversees creation and submission of annual CMS Medicare Part D MTM Program description.
- Ensures specifically that all requirements to satisfy Medicare Medication Therapy Management Program (MTMP), including policies and procedures are in place and functioning to assure complete compliance and audit readiness.
- Develops written policies and procedures for the delivery of clinical pharmacy services and programs.
- Participates in development of strategies to improve medication optimization that support the triple aim.
- Participates in development of programs to manage pharmacy and total cost of care trends and quality measures with continuous improvements.
- Participates as a member of health plan and medical group committees or subcommittees as needed (e.g., Pharmacy and therapeutics, Institutional Review Board, etc.).
- Meets with employers as needed to share MTM program results. (30%) -Leads, manages, and develops a team of professional and clinical staff including having the authority to hire, transfer, lay off, promote, discipline and discharge, train, reward, and review performance of employees.
- (10%) – Serves as Residency Director for PGY1 Managed Care Residency Program.
- (5%) - Manages MTM program budget.
