About the Role
The Care Coordinator at Jasper Health plays a crucial role in supporting cancer patients by facilitating access to essential supportive services and social resources. This position involves collaborating with interdisciplinary teams, conducting consultations, initial assessments, and developing personalized care plans to address the holistic needs of patients undergoing cancer treatment. The Care Coordinator serves as a vital link between patients, caregivers, and Jasper’s supportive care counselors, ensuring seamless coordination of care and optimal patient outcomes.
Role and Responsibilities
- Engage with patients through consultations to explain program expectations and goals
- Utilize technology to document interactions, track tasks, and communicate effectively with the counseling team
- Execute and optimize scheduling and care coordination to ensure positive patient experience and clinical team efficiencies
- Ensure timely follow-up on care coordination needs and progress of care plans
- Diligent follow-up with patients to ensure appointment attendance and follow-up
Qualifications and Education Requirements
- 2+ years of relevant experience in healthcare or social services
- Proficiency in utilizing technology for remote patient support
- Familiarity with electronic health record systems and scheduling platforms
- Strong communication and problem-solving skills
- Ability to adapt to changing environments and prioritize tasks effectively
Conditions of Employment
