Position Description

Anoka County Community Social Services and Behavioral Health (CSS/BH) currently has one full-time position available as a Senior Social Worker. Adult mental health intake provides intake and referral services to individuals seeking community based mental health services or mental health case management.

This is a full-time, exempt, hybrid position. Interviews will take place the week of March 17th, 2025 for those selected to move forward in the hiring process. 

Pay & Benefits

  • Salary: $85,363 to $102,440 ($41.04 to $49.25 per hour).
  • 2025 Anoka County Salary Schedule Grade 36: $85,363 to $119,496 ($41.04 to $57.45 per hour).

Work Location

  • This position will work at Anoka County Government Center, located at 2100 3rd Ave, Anoka, MN 55303.
  • This position may be eligible for flexible work arrangements, including hybrid work, with some days working remotely and some days working in the office.
  • Expected work hours are Monday – Friday, 8:00 a.m. – 4:30 p.m.

Job Duties and Responsibilities
These examples are not all inclusive and are intended to be illustrative of primary responsibilities of an Anoka County Social Worker.

  • Complete OBRA Level II assessments and evaluative reports in a timely manner.
  • Screen, assess, plan, refer, and provide early intervention case management to adult clients referred to the department for services.
  • Conduct outreach, and monitor services designed to meet the individualized mental health needs of adults.
  • Complete functional assessments and individual community support plans that outline the identified service needs, resources and required monitoring for assigned caseload.
  • Make monthly face to face visits with assigned adult clients in their home, or in various community settings as appropriate.
  • Support adult clients as they strive to understand how to access or navigate community services that are evidence based, clinically appropriate, and culturally responsive.
  • Help adult clients access county programs and community-based services such as home-based therapy, individual therapy, family therapy, intensive outpatient treatment, partial hospitalization programs, crisis stabilization services, and medication management, that help meet their service needs.
  • Generate resource and referral information on the continuum of services for adult clients and share information as needed with individuals, families/guardians, agencies, and communities to help identify unmet needs in the system.
  • Assist adult clients with the process of obtaining community based least restrictive mental health care, and/or obtaining higher level of care as needed.
  • Using and navigating SSIS and OnBase for timely completion of needed paperwork, case notes, court and/or legal documents, electronic files.
  • Facilitate clinical screenings, case consultations, case transfers, and/or multidisciplinary team meetings as needed.
  • Participate in individual and group clinical supervision in compliance with state statute.
  • Attend and participate in continuing education, training, or in service offering required to maintain license in good standing.
