Schedule: Monday-Friday 9:00 a.m. - 1:00 p.m.

Job Summary: The Housing Solutions Case Manager provides ongoing needs assessments, emergency and housing plans, support, referrals, and advocacy for individuals in the Mercy House Permanent Supportive Housing Program. This role includes facilitating life skills workshops, community-building events, and ensuring residents are supported in achieving self-reliance and housing stability. Strong documentation, trauma-informed care, and client-focused strategies are critical.
Essential Duties and Responsibilities:
Program Oversight
- Manage a caseload of 20–22 residents; meet at least twice monthly to update individual housing and emergency plans
- Conduct in-person meetings at client homes or office; perform home visits and unit inspections
- Use harm reduction, crisis intervention, trauma-informed care, motivational interviewing, and eviction prevention techniques
- Support residents in identifying strengths and addressing challenges respectfully
- Refer clients to appropriate community resources and coordinate support services
- Facilitate social/recreational activities and foster community engagement and peer support
- Collaborate with volunteers and outside services (employment, education, mentoring)
- Build positive landlord-client relationships and participate in emergency on-call rotation
- Attend relevant trainings to improve service delivery
Residential Services
- Promote client participation in credit counseling, job search, financial literacy, and computer training
- Plan and lead life skills workshops (e.g., stress/time management, wellness, communication)
- Use survey feedback to develop enrichment and community-building events that meet program goals
- Coordinate job readiness sessions and support residents in building social networks
- Organize holiday and community events; maintain and distribute monthly event calendars and quarterly newsletters
- Order necessary supplies for resident activities and provide reports to management
Team Collaboration
- Actively participate in agency meetings and initiatives, aligning with Mercy House’s mission and values
- Collaborate with property management on leasing and tenancy to ensure effective communication and service delivery
- Support a positive and inclusive community environment for all residents
Administration/Miscellaneous
- Maintain accurate, timely documentation including case notes, plans, assessments, and Housing Authority paperwork (applications/recertifications)
- Ensure timely completion of agency paperwork and incident reports
- Assist with data collection and reporting, including use of Coordinated Entry System and HMIS
- Coordinate intake and move-in documentation; support ongoing eligibility and recertification processes
- Collaborate with on-site property staff; meet all assigned deadlines
- Develop knowledge of Adult Rehabilitative Mental Health Services, substance abuse treatments, and fair housing practices
- Respond promptly to communication; maintain confidentiality and adhere to HIPAA
- Prioritize workload, work independently, and uphold ethical case management practices
- Perform additional duties as assigned by management
Qualifications
Requirements:
Education & Experience
- Bachelor’s degree in social work or related field is preferred, with experience in recovery, mental health, or co-occurring disorders
