Case Manager Assistant

JOB SUMMARY:
To provide administrative support to the clinical team.
RESPONSIBILITIES:
· Triage phone calls from patients, families, doctors and vendors. Follows up on issues that can be resolved without the assistance of the Case Manager in a timely matter while communicating to the Case Manager via email.
· Consults with other departments to resolve issues regarding HHA services, scheduling issues and supply orders.
· Reports any changes regarding the patient to the Case Manager and Clinical Manager same day as reported by patient/caregiver.
· Communicates all actions taken on behalf of the Case Manager to the Case Manager and his/her Clinical Manager via email on the same day received and completed.
· Reports to Case Manager regarding any issues that must be resolved by the Case Manager that are not time sensitive and corresponding to the Clinical Manager via email the same day.

· Calls and sends follow up note to Case Manager and Clinical Manager as appropriate for time sensitive issues, and issues that require immediate attention from the Case Manager.
· Create skilled nursing orders as well as HHA Plan of Care based on nursing collaboration note under direction of the Case Manager.
· Create 485 based on nursing collaboration note under direction of the Case Manager.
· Coordinate with HHA scheduler for any HHA issues same day as reported issue/ and or complaint.
· Other duties as assigned.

QUALIFICATIONS:
1. High School Diploma, and or a minimum of one year of experience in home care preferred.
2. Demonstrates competency in Microsoft Word, Outlook, and Excel programs.
3. Competent in McKesson operating system.
4. Excellent written and verbal skills required.
5. Possess positive attitude, good people skills and professional appearance.
6. Ability to work independently without constant supervision.
Salary: $28/HR
