Job Summary

Under general supervision, the Hospital Care Management Project Coordinator is responsible for facilitating and coordinating a variety of patient facing projects and department business functions. Coordination involves assessment, planning, support and evaluation of patient related outcomes associated with identified projects. Activities to be performed are contract management, scheduling and planning department meetings, coordination of projects which include tracking of progress, data analysis, maintain employee records and liaise with internal and external community partners to facilitate and support the Hospital Care Management departmental operations within the organization. The Hospital Care Management Project Coordinator will interface with internal and external providers and customers.

Essential Functions:
  • Provides administrative support to the HCM Director and Manager. (20%)
  • Maintains employee coverage calendar, department folders and employee files. (15%)
  • Promotes positive customer service with internal and external relationships. (10%)
  • Coordinates department functions and strategic planning projects. (10%)
  • Conducts special marketing projects and outreach as assigned. (10%)
  • Conducts follow up calls and assessments to alumni of the ACUTE program. (10%)
  • Provides necessary support and intervention to alumni as needed. (10%)
  • Documents contact and necessary intervention in the medical record in a timely and accurate way. (5%)
  • Actively participates in assigned meetings. (5%)
  • Promotes positive customer service with both internal and external relationships. (5%)
  • Adheres to, complies with and demonstrates support to the mission and values of Denver Health. (0%)
  • Ensures confidentiality of patient information by creating and maintaining a secure and trusting environment by not sharing information learned on the job, except when necessary in the job performance of the job responsibilities or to improve a patient’s care. (0%)

Education:
  • High School Diploma or GED Required

Work Experience:
  • 1-3 years Project/Program coordination or Case management experience within the medical field. Required

Licenses:
  • BLS-Basic Life Support (BLS/CPR) - AHA - American Heart Association Required

Knowledge, Skills and Abilities:
  • Knowledge and understanding of case management/coordination of care principles, programs, and processes in either a hospital or outpatient healthcare environment required.
  • Effective problem-solving skills, including the ability to analyze complex situations, draw conclusions and implement actions appropriately and efficiently.
  • Evidence of leadership, creativity, integrity and initiative required.
  • Ability to work effectively in a team environment.
  • Experience with windows-based computer programs and ability to use computer for data analysis and data display required.
  • Must be able to type 35 words/minute.
  • EPIC experience a plus.
