Job Summary:

The Registered Nurse (Home Health RN) responsibilities include performing Medicare  SOC, discharge, patient visits and monitoring their conditions, conducting tests, and writing reports for the physician.

Job Responsibilities:

  -The  Case Manager (Home Health RN)'s responsibilities include performing patient visits, monitoring their conditions, conducting tests, and writing reports for the physician."

· Traveling to patients’ home, caregiver's concerns and managing their care plans according to physician's instructions.

· Administering medication and insulin, and completing blood pressure, glucose, urine, and laboratory testing.

· Inspecting wounds, changing dressings, and handling personal grooming and hygiene.

· Testing for muscle weakness, bedsores, and any signs of infection.

· Listening to the concerns of family members and answering their questions.

· Educating caregivers and family on the aftercare or ongoing care of the patient.

· Providing suggestions for improved healthcare to physicians and family members of the patient.

· Monitoring patient recovery and compiling reports for the physician.

· Collaborating with doctors and other healthcare professionals to develop improved diets and healthcare plans for patients.

Requirements

Experience:

  • One (1) year experience as an RN in a clinical care setting. 
  • Preferred, Home health experience. 

Skills:

  • Oasis
  • Nursing skills as defined as generally accepted standards of practice. 
  • Excellent interpersonal skills and ability to communicate effectively. 
  • Demonstrates proven decision-making skills. 
  • If required to make visits, 2 step TB, chest x-ray, or QuantiFERON prior to visits  
  • Must read, write and comprehend English.

Education:

  • Completion of an accredited Registered Nursing Program. 
  • Preferred, BSN.

Licensure/Certification:

  • Current driver’s license in good standing. It is the responsibility of the employee to renew their driver’s license before it expires in order to continue employment. 
  • Reliable transportation and auto liability insurance.
  • Current State professional license as an RN in any/all applicable states. 
  • Current CPR.

On-going employment as an RN requires maintenance of a valid RN license in the state which the nurse practices in and a valid CPR certificate at all times. It is the responsibility of the employee to renew these both before they expire in order to continue employment. 

Physical Requirements:

  • Prolonged standing/walking required.
  • The ability to lift/transfer up to 50 lbs.
  • Ability to meet deadlines and patient needs, make quick decisions and allocate resources under stressful conditions.
  • Meet patient/family individualized psycho social needs.
  • Requires hand-eye coordination and manual dexterity.

Environmental/Working Conditions:

  • Works in patient’s home in various conditions.
  • Possible exposure to blood borne pathogens, bodily fluids and infectious diseases.
  • Some exposure to unpleasant weather; PRN emergency call.
  • Ability to travel locally and work a flexible schedule.
  •  On-Call Rotation once training period completed required for all full time staff, optional for PRN or Part Time staff
