Weekly Hours: 35.00 

Exemption Status: Exempt

Salary Range: $85,200.00 - $93,100.00  

*As required under NYC Human Rights Law Int 1208-2018 - Salary range for this role when Hired for NYC Offices 

Position Summary

The clinical behavioral health care specialist will work with a clinical team that advances behavioral health integration in the pediatric primary care setting using principles of the Collaborative Care Model. 

Job Responsibilities

  • Provides evidence-based behavioral health interventions and psychoeducation for youth with a broad range of presenting concerns (i.e. anxiety, depression, behavioral problems).
  • Assesses and systematically collects and tracks data in a patient registry and works with the team to develop a plan of care.
  • Communicates with pediatricians about plan of care; participates in huddles to discuss shared patients.
  • Utilizes data, including registries, to continuously meet care coordination needs across settings in conjunction with the patient, family and/or other care providers.
  • Develops and maintains a working knowledge of community resources and referrals.
  • Assumes responsibility and accountability for effectively managing behavioral health care of individual patients.
  • Documents history, assessments, and interventions to reflect comprehensive and integrated approach to patient care. Ensures complete, accurate and timely documentation in the electronic medical record.
  • Attends team meetings and case conferences to present and discuss patient disposition and outcomes data.
  • Assists with the coordination of care across the continuum (e.g. inpatient, outpatient, home, schools).
  • Assists with management of urgent and emergent patient care situations.
  • Assists with the management of patients with chronic health conditions, acute episodes of care, or behavioral health challenges while promoting prevention and wellness.
  • Provides care that is safe, ethical, patient- and family-centered, culturally congruent and evidence-based.
  • Performs other related duties as assigned and as appropriate to scope, education, and/or training.

Education

  • Master's Degree

Experience

  • Must have training and experience working with children, adolescents and families with mental health concerns
  • Experience providing evidence-based psychotherapeutic approaches; cognitive-behavioral therapy experience preferred.

Knowledge, Skills and Abilities

  • Ability to thrive within a complex medical system and working understanding of the healthcare system.

Licenses and Certifications

  • Licensure in social work (LCSW or LMSW) or mental health counseling (LMHC). 

Working Conditions/Physical Demands
