The RN Case Manager (RN CM), works in collaboration with a multidisciplinary team by identifying and initiating discharge planning to accomplish the timely transition of patients to a post-acute hospital setting and to ensure patients progress along the continuum of care. RN CM facilitates patient transitions efficiently and in a cost-effective manner that ensures quality outcomes. RN CM utilizes established guidelines to support appropriate level of care throughout hospitalization in compliance with CMS and organizational utilization management requirements. RN CM helps to ensure medical necessity is met by regularly reviewing severity of illness and intensity of service for patients to ensure appropriate and timely utilization of hospital services.Licensure/Certification/Registration: Current GA RN license
Education: Must be a graduate of an accredited school of nursing, BSN recommended
Experience: Must have a minimum of 2 years experience in nursing and a minimum of
1 year in case management, utilization management, or discharge planning preferred. Certification in case management preferred.
Special Qualifications: Excellent communication, interpersonal, and critical thinking skills.
Degree of Supervision Required: Indirect and periodic supervision. Must be capable of working independently.

This position requires competence in assessment, treatment, and/or care for the age groups indicated. The staff member must be able to demonstrate the knowledge and skills necessary to provide care, based on physical, psycho/social, educational, safety and related criteria, appropriate to the age of the patients served in her assigned area. The skills and knowledge needed to provide such care might be gained through education, training, or experience.
