About the role

Job Title: Care Coordinator - (PART-TIME)


 
  • In this position, the care coordinator will be partnered with the radiation oncology social worker and is responsible for coordinating care and providing guidance to oncology patients of differing diagnoses undergoing radiation treatment. 
  • They will work in collaboration to assist and support patients and their families throughout the cancer journey. 
  • The care coordinator will also be responsible for developing effective relationships with patients and their family caregivers, and working closely with the interdisciplinary care team to improve health outcomes. 
  • The care coordinator will play a critical role within the multidisciplinary team by building working relationships, engaging in problem solving, and coordinating/connecting patients with resources. 
  • The care coordinator will assist the medical team with frequently encountered barriers to quality health care including: financial difficulties, communication problems (such as lack of understanding, language/cultural), medical system management (easy access to appointment scheduling, follow up to missed appointments), and functional issues (such as transportation, home care, and other oncology resource referrals). 
  • The care coordinator will serve as a patient advocate by providing attainable alternatives or solutions to aforementioned barriers, make appropriate referrals and document care in designated system. 

 

Work days (3 days a week): TBD

  • Bachelor’s degree. 
  • Experience in healthcare or related field is preferred.
  • Excellent oral and written communication skills and ability to foster a supportive environment for patients.
  • Bilingual (English/Spanish) preferred. 
  • Minimum of one year in current position.

 

 

Compensation Statement

 

The Mount Sinai Health System (MSHS) provides a salary range to comply with the New York City Law on Salary Transparency in Job Advertisements. The salary range for this role is $58,661.00 - $74,853.45 Annually. Actual salaries depend on a variety of factors, including experience, education, and hospital need. The salary range or contractual rate listed does not include bonuses/incentive, differential pay or other forms of compensation or benefits.
